  • The following duties are normal for this position. Other duties may be required and assigned.
  • Assists field staff in meeting the qualifications to deliver quality care to the clients.
  • Assists in staff education including orientation, proficiency check-offs, and annual education programs as assigned by the upper management.
  • Supervises and exhibits leadership capabilities and provides guidance and instruction to personal support staff as needed.
  • Monitors field staff performance to assure adherence to insurance and state regulations.
  • Performs client admissions timely and accurately, as required. Solicits necessary client specific information to support the client’s admission to the agency as set forth by PSSA regulations.
  • Regularly visits client homes to observe caregiver interactions, monitor client well-being, address any concerns raised by clients or caregivers, and ensure adherence to care plans.
  • Performs supervisory visits to clients’ homes in a timely manner and submits related documentation timely. Visits will not exceed once per month nor be less than once per quarter.
  • Uses initiative and self-direction to schedule admissions and supervisory visits the day prior.
  • Reports negative comments, complaints, concerns/issues discovered during home visits, to upper management in a timely manner.
  • Assists in the investigation of critical incidents as directed by upper management and documents findings in a timely manner.
  • Assists in providing disciplinary action to field employees as needed and directed from upper management.
  • Submits records and reports timely as required by administration. Submits paperwork that is neat, legible, and written in dark colored ink or electronically, as required.
  • Performs field staff performance/competency evaluations within 30 days of employment and at least yearly thereafter, at the direction of office staff.
  • Ensures compliance with all applicable company, insurance, state, and federal rules and regulations.
  • Notifies the office of any anticipated problems with the posted schedule. Provides as much advance notice of changes/problems as is possible.
  • Adheres to agency infection control and safety policies, including education, reporting and practice implementation specific to job position.
  • Always maintains a professional appearance. Follows dress code policy requirements.
  • Maintains confidentiality of all records and reports maintained for all clients.

Mathematical Skills: Ability to calculate figures and amounts such as proportions, percentages, area, and volume. Ability to apply concepts of basic algebra and geometry.

Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Must be able to exercise good judgment in decision making.

Environmental Demands: Ability to adapt and perform job duties in demanding environments (i.e. temperature changes, cigarette exposure, pet exposure, and various other household chemicals).

Physical Demands: Ability to regularly stand, walk, use hands to handle/feel, reach with hands and arms, stoop, kneel, crouch, crawl, talk, hear, smell, manipulate up to 50 pounds, lift up-to 30 pounds, and ability to walk independently up and down a normal flight of stairs. Specific vision abilities could include close vision, distance vision, color vision, peripheral vision, and ability to adjust focus.

Transportation Demands: Must have a valid driver’s license and/or the availability of an insured vehicle to travel to clients’ homes and agreed upon meeting places.

  • Work with patients in a hospital setting and provide them with an array of services
  • Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes
  • Maintain the highest standards of professional conduct
  • Deliver care in a manner that protects and preserves the patients dignity, rights, values, beliefs, privacy and autonomy.
Qualifications:

Job Requirements

This job requires a person who can work independently, yet never forget that continuous care requires a team effort. Additional requirements include:

  • You must have a valid State nursing license or a license from a compact state.
  • At least 2 year of nursing experience in your specialty
  • Ability to cope with the emotional needs of patients and their families
  • Ability to communicate effectively, both verbally and in writing
  • Respecting the patient's privacy and maintaining confidentiality
  • Current TN driver's license and automobile liability insurance
  • BLS, PALS & ACLS Certification (Depending on floor)
  • Current TB skin test
  • You must be able to satisfy employee screening results (background check, drug testing, motor vehicle report, licensure verification, abuse registry, OIG excluded individuals)
  • 2 letters of recommendation, resume & immunization records
  • 2 References
  • Pass a routine physical (Depending on accepted facility)
