Job Description
We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our home health team on a part-time basis. This role focuses on evaluating and treating patients with speech, language, cognitive, and swallowing disorders in the comfort of their homes.
The ideal candidate is patient-centered, flexible, and committed to improving quality of life through individualized therapy.
Responsibilities
- Evaluate and assess patients with speech, language, cognitive, and swallowing disorders
- Develop and implement individualized treatment plans
- Provide therapy in patients’ homes in accordance with physician orders
- Educate patients and caregivers on therapy techniques and home exercises
- Collaborate with nurses, physicians, and interdisciplinary team members
- Document evaluations, progress notes, and discharge summaries accurately and timely
- Monitor and adjust care plans based on patient progress
Qualifications
- Current Speech-Language Pathologist (SLP) license in the state of [State]
- Master’s degree in Speech-Language Pathology
- Certificate of Clinical Competence (CCC-SLP) preferred
- Minimum 1 year experience (home health or hospice preferred)
- Strong communication and clinical skills
- Ability to work independently
- Valid driver’s license and reliable transportation
