Compensation: $46,000.00 - $63,000.00 / hour
Fosters meaningful connections between Astrana Health and the community we serve. This position is responsible for developing and maintaining partnerships with community-based organizations to support membership retention and growth. The specialist will lead community engagement efforts, help coordinate events and implement outreach strategies. What You’ll Do:
• Build and sustain strong relationships with community-based organizations to promote membership growth and retention. • Lead community engagement initiatives that enhance the organization’s visibility and trust within diverse communities. • Plan, organize, and execute community events, including: o Senior expos and health fairs o Doc Talks/Lunch & Learns o Sponsorships o Astrana hosted events (fun social events) o SWAG Center Activities • Track lead generation from grassroots activities and ensure timely follow-up to convert leads into members. • Maintain monthly event invite email blast through Campaign Monitor or another platform. • Collaborate with team to develop and implement outreach and marketing strategies tailored to senior population. Qualifications:
• Associate's or Bachelor’s degree in Healthcare Administration, Social Work, Public Health, or a related field preferred but not required.
• 2–4 years of experience in patient engagement, care coordination, or healthcare customer service.
• Strong interpersonal and communication skills with the ability to build rapport and trust with diverse patient populations.
• Familiarity with healthcare systems, medical terminology, and insurance navigation (Medicare and Medicaid knowledge is a plus).
• Bilingual (Spanish or other languages) preferred but not required.
• Proficiency in EHR systems and administrative platforms; ability to document accurately and manage multiple systems simultaneously.
• Demonstrated ability to manage time effectively, multitask, and problem-solve in a fast-paced environment.
• Passion for patient-centered care and improving health outcomes in underserved communities.

- The national target pay range for this role is $46,000 - $63,000. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.
