POSITION SUMMARY

The Case Manager – Traditional CBS provides goal-directed, medically necessary services to children and youth with Serious Emotional Disturbance (SED) and their families. Working primarily in community settings, the Case Manager delivers face-to-face, solution-focused interventions designed to help persons served reduce the effects of mental and emotional impairments, improve daily functioning, and enhance community integration. Responsibilities include care coordination, advocacy, and assisting clients in accessing medical, educational, and social services. All interventions are guided by individualized treatment plans approved by a Qualified Mental Health Provider with the aim of promoting recovery, stability, and reducing the need for institutional care.

ESSENTIAL FUNCTIONS

  • Provide goal-directed, strength-based, and recovery-oriented services in community settings (home, school, etc).
  • Collaborate with clients, families, and treatment teams to develop and monitor individualized treatment and crisis plans.
  • Deliver supportive counseling, emotional and behavioral interventions, skill-building, and crisis response.
  • Coordinate services with schools, healthcare providers, social services, and other community resources.
  • Assist with benefit applications, transportation arrangements, and accessing essential services.
  • Conduct strengths and needs assessments, complete and update treatment plans and clinical documentation.
  • Serve as an advocate for clients and families and act as a liaison across systems of care.
  • Participate in internal meetings and external networking to enhance service access.
  • Maintain confidentiality, promote cultural competence, and adhere to trauma-informed practices.
  • Actively support and promote the agency's mission and strengths-based philosophy.
  • Complete required background checks every two years.
  • Complete required training.
  • Work as part of an integrated treatment team collaborating with the individual served, family members or others they want involved in their treatment and other treatment team members (internal and external) to help support the individual in their treatment and recovery.
  • Demonstrate cultural and linguistic competence.
  • Approach interactions with individuals served and colleagues with a trauma sensitive lens.
  • Provide services that are recovery oriented, and person and family centered, utilizing a strengths-based approach.
  • Provide services to meet the whole health needs of individuals served.
  • Utilize appropriate assessment instruments to help monitor the progress of individuals served and ensure necessary services are received in the appropriate intensity and duration.
  • Utilize Evidence Based Practices (EBPs) as appropriate.
  • Other duties as assigned by respective team managers or supervisors.

POSITION QUALIFICATIONS

Education/Licensure:

  • BA/BS degree in Human Services (preferred) or equivalent combination of education and/or a minimum of 4 years of relevant work experience in a related field may be considered.
  • Highschool Diploma required.

Experience:

Prior experience working with children, adolescents, and families preferred.

Requirements:

  • Must successfully pass the KBI, Drug screen, and all DCF Central Registry background investigations.
  • Valid driver's license, current auto insurance, a good driving record and a reliable vehicle are required.

WORK ENVIRONMENT & PHYSICAL DEMANDS

  • Office and community settings, including client homes and schools
  • Exposure to individuals in crisis, high-stress environments, and economically disadvantaged communities
  • Occasional physical activity, including standing, walking, bending, or intervention in behavioral crises (e.g., MAB).
  • Frequent travel within assigned region

SUPERVISORY RESPONSIBILITIES

This position has no direct supervisory responsibilities but does serve as a coach and mentor for other positions in the department.

Other Physical Requirements:

Case Managers must have sufficient ambulatory capabilities to work with children and adolescents in the community-based setting. Position may require the ability to use physical holding (MAB) with children and adolescents exhibiting out of control behaviors.
