Duties
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods
- Monitor patient health by taking vital signs, observing for changes in condition, and accurately documenting care plans in compliance with HIPAA regulations
- Support medication administration under supervision or according to care plans, ensuring safety and adherence to protocols
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Provide personal transportation assistance for appointments, community outings, or errands to promote engagement and independence
- Maintain a clean and safe environment through regular cleaning duties and infection control practices aligned with healthcare standards
- Support residents with cognitive impairments such as dementia or Alzheimer's disease by implementing memory care strategies and engaging in meaningful activities
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated ability to observe patients closely and respond promptly to changes in condition or behavior
- Knowledge of patient monitoring techniques, care plans, medication administration, and vital signs measurement
- Experience working with individuals with developmental disabilities, neurodevelopmental disorders, autism spectrum disorder (ASD), or intellectual disabilities support is advantageous
- Familiarity with HIPAA compliance, patient personal care assistance, and manual handling equipment such as Hoyer lifts
- Certification in CPR and First Aid is required; additional certifications related to hospice care or geriatrics are a plus
- Strong communication skills for effective client interaction and collaboration with healthcare teams
