Support Specialists
Our client is contracted with multiple insurance agencies to partner with them on assisting their members with kidney disease. Our client has nurse and social worker employees that visit the members in their homes to assist them with their needs. Their needs could be anywhere from assisting them with transportation, prescription consolidation, dialysis/connecting them with a doctor, nutrition, to anywhere up to helping them with their quality of life in their home, etc.
- The Support Specialist will be given a list of referrals from the health insurance company to call and schedule in home appointments for. (members with kidney disease)
- Their goal is to schedule the in home appointment for the member.
- They will be utilizing only 1 system (salesforce) to enter in all their notes and follow through to make sure the appointment is set and everything goes smoothly.
- Works closely with the nurses and social worker on their care team to provide an excellent experience for their patients.
- Coordinate home visit schedules based on location and availability
- Contact physician offices and other providers by phone, fax and email in order to arrange patient appointments or coordinate the sharing of clinical information with them when appropriate and request they share information back with us, including facilitating the secure transmission of that information
- Field inbound calls with a positive attitude, building rapport and trust with patients
- Provide support and troubleshooting for patients who need assistance with biometric devices
- Enter data as needed into Salesforce Health Cloud or any other system, as required
- Support Monogram Health as necessary with any tasks required to deliver excellent personalized kidney care and perform all other duties as assigned
Performance Expectations:
- Will be expected to handle 90-100 Inbound & outbound calls calls a day
- Will be expected to handle member conversations with strong customer service and be able to handle rebuttals well
- Will be expected to have strong attendance (coming to work on time every day, no call outs unless it's an emergency during the contract period, ensuring all doctors appts/planned mandatory events are expressed to the manager at least 1-2 weeks in advance)
- Maintains the Nurse Schedule and focus on Patient Retention.
- These patients are already enrolled in the services.
- This role has a ton of follow up
- Scheduling existing Patients (white glove service)
Skills & Qualifications:
- HS Diploma or GED
- 2-3 years of call center or scheduling experience required
- Healthcare call center background is a plus but not required
- Must be tech savvy and able to toggle between multiple systems simultaneously (salesforce, phone, email, etc.)
- Must have strong tenure (at least a year in most recent positions)
- Bilingual preferred!!!
- Must have strong empathy as they are dealing with primarily senior citizens as the main demographic
Details:
Start Date: May 15th, classes will continue until we fill this!!
Pay: $20/hour, $21/hour if bilingual
Vaccine required, exemptions allowed
Schedule
- Training: Monday – Friday 8am – 5pm for 1 week
- Regular schedule:
- Monday - Friday
- 6am – 3pm
- 8am – 5pm
