We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA) to provide high-quality, person-centered care to clients in their homes. The ideal candidate will possess a strong background in personal care and assisting with activities of daily living (ADLs). This role offers an opportunity to make a meaningful difference in the lives of individuals with diverse needs, including those requiring memory care, dementia support, and developmental disabilities. Duties

• Assist clients with personal hygiene, grooming, dressing, and mobility assist (if/ when needed) to promote independence and dignity.
• Monitor client’s health status by observing patient behavior and overall well-being; report any changes promptly.
• Provide assistance with meal preparation, feeding, and ensuring proper nutrition tailored to individual dietary needs.
• Utilize transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility for clients.
• Engage clients in meaningful conversation, activities, and social interactions to enhance their quality of life.
• Maintain cleanliness of client homes, through routine cleaning tasks aligned with your client’s plan of care and health standards.
• Follow established care plans for clients with Alzheimer’s disease, dementia, or neurodevelopmental disorders, ensuring specialized support is provided.
• Assist with transportation to medical appointments or community outings while always ensuring safety.
• Document care activities accurately in compliance with healthcare regulations and Assured Healthcare policies.

Experience

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred.
• Demonstrated ability to perform patient observation and monitoring in various care environments.
• Experience working with individuals with autism, developmental disabilities, or intellectual disabilities support is advantageous.
• Knowledge of dementia care, memory care techniques, hospice care principles, and geriatrics is desirable.
• Familiarity with care plans and tasks.
• Certification in CPR and First Aid is required; ongoing training in patient personal care assistance is beneficial.
• Proven ability to handle heavy lifting safely using proper patient transfer methods such as Hoyer lifts or manual handling skills.
