RN Clinical Manager - Homecare

Registered Nurse

1. Receives and coordinates case referrals. Reviews available patient information related

to case, including disciplines required, to determine home care needs. Assigns

appropriate clinicians to case.

2. Facilitates, coordinates and integrates orders from all physicians.

3. Reviews and evaluates each case by reviewing the services provided by clinicians,

conferences, record review, discusses and verifies impressions, instructs and guides

clinicians to promote more effective performance and delivery of quality home care

services. Assures the development, implementation and update of the individualized

plan of care.

4. Available at all times during operating hours to assist clinicians as appropriate.

5. Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

6. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting

priorities, and in developing plan of care.

7. Reviews and critiques all initial clinical staff documentation. Assures accuracy and

appropriateness.

8. Attends and facilitates case conference meetings with agency personnel to facilitate

coordination of care.

9. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate agency personnel.

10. Assists in the screening and interviewing process of new agency personnel and makes

recommendations for employment of individuals. Assists in the orientation of new

agency personnel.

11. Assists Clinical Director in the planning, implementation and evaluation of in-service

and continuing education programs.

12. Participates in agency Quality Assessment Performance Improvement and Performance Improvement Projects as assigned.

13. Performs other duties and activities as delegated by the Director of Clinical Services.
