Duties
- Provide compassionate assistance with activities of daily living such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates
- Assist clients with medication reminders and ensure proper medication administration following medical documentation protocols
- Observe patients closely for changes in condition or behavior, especially in cases involving dementia or Alzheimer's care
- Support clients with meal preparation, feeding, cleaning, laundry, and light housekeeping to maintain a safe and comfortable environment
- Document all patient interactions and care activities using Electronic Health Record (EHR) systems while adhering to HIPAA privacy standards
- Use medical terminology correctly when recording notes or communicating with healthcare professionals
- Employ specialized skills such as catheter care, hospice support, working with individuals with developmental disabilities or disabilities, and managing memory care needs
Requirements
- Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments
- Knowledge of medical terminology and familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
- Ability to work independently while following detailed care plans and instructions from healthcare providers
- Strong patient observation skills to identify changes in condition or behavior promptly
- Experience working with individuals with disabilities or developmental challenges preferred
- Excellent communication skills to interact effectively with clients and their families while maintaining confidentiality under HIPAA regulations
- Physical ability to perform caregiving tasks including lifting, transferring using Hoyer lifts, standing for extended periods, and performing light housekeeping duties
