Job Overview

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in physical therapy practices, particularly in home health settings. This role involves working closely with patients to develop personalized treatment plans that promote recovery and enhance mobility. As a Physical Therapist, you will play a crucial role in the rehabilitation process, ensuring that patients receive the highest quality of care. This position is 1099/independent contractor, servicing Sacramento and Placer Counties.

Pay rate: $120-$145 per visit

Duties

• Assess patient conditions through comprehensive evaluations and medical history reviews.
• Develop individualized treatment plans based on patient needs, goals, and medical conditions.
• Implement therapeutic exercises and modalities to improve patient strength, flexibility, and mobility.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Collaborate with interdisciplinary teams including physicians, nurses, and other healthcare professionals to coordinate care.
• Educate patients and their families on treatment plans, exercises, and strategies for recovery.
• Document patient progress accurately in compliance with healthcare regulations and standards.
• Participate in discharge planning to ensure continuity of care post-treatment.

Skills

• Strong knowledge of medical terminology related to physical therapy practices.
• Proficiency in anatomy and physiology relevant to rehabilitation techniques.
• Experience with home settings.
• Familiarity with pediatric physical therapy practices is a plus.
• Ability to effectively communicate with patients, families, and healthcare team members.
