Job Description
Responsible for working closely with hospitals and other facilities to coordinate continued patient care with hospice home healthcare services after discharge. Assures quality customer service to patients, families, referral sources and co-workers. Serves as a consultant, educator, and customer service specialist to referral sources.
Principal Accountabilities:
- Develops initial plan of care for a Hospice admission to Home Hospice, or to a General Inpatient bed.
- Coordinates with patients, families, physicians, and discharge planning personnel, by obtaining complete clinical information for the hospice admission.
- Contacts the Hospice Manager, as needed to review the appropriateness of General Inpatient Hospice.
- Communicates the levels of care for Hospice to the patients and families, and have the patient admitted to the appropriate level of care.
- Obtains an initial Hospice order from the referring physician and contact one of the Hospice Medical Directors for certification of terminal illness, for admission to General Inpatient Hospice.
- Maintains current knowledge of clinical issues, regulatory and reimbursement standards, insurance contracts and negotiated rate amounts.
- Performs other related duties as assigned.
Qualifications for Internal Candidates
- Bachelor's degree in Nursing
- License for Registered Nurse.
- Clean driving record
- BLS certification
- A completed apprenticeship or experience in a similar role.

Hospice
At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace. Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers. As one of the first hospices in New Jersey and the United States, we have more than 40 years' experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties. When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate.
