    Provide physical therapy services to patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes.     Join Vanderbilt Health Home Care Services, where compassionate care meets clinical excellence in the comfort of patients' homes. We are seeking a dedicated Physical Therapist (PT) to join our Home Health team in a full-time capacity, serving patients across the Greater Nashville Area.

This Home Health PT position provides autonomy, weekday flexibility, and the support of a nationally recognized health system.     Key Responsibilities

    • Provide high-quality, evidence-based physical therapy in patients' homes
    • Deliver care according to individualized treatment plans developed by supervising PTs
    • Promote patient safety, mobility, and functional independence
    • Educate patients and caregivers on exercises, mobility strategies, and home safety
    • Accurately document clinical notes and patient progress in EMR systems
    • Communicate effectively with interdisciplinary team members
    • Travel independently to patient homes within designated service areas

    Required Qualifications

    • Active Physical Therapist (PT) license in the state of Tennessee
    • Minimum 1 year of PT experience
    • Strong communication and assessment skills
    • Ability to work independently with a flexible caseload
    • A valid driver's license, reliable transportation, and ability to maintain a satisfactory driving record

    Preferred Qualifications

    • Home Health experience
    • Experience with EMR documentation systems
    • Strong understanding of rehabilitation principles and functional mobility

    PLANNING AND MANAGING CARE

    • Formulate functional treatment plan that is goal focused, individualized, safe and age-specific to address deficits and patient/caregiver areas of concern.
    • Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.
    • Administer and score standardized and criterion referenced tests.
    • Interpret the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.

    • Provide therapeutic interventions for identified objectives.
    • Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.
    • Assure coordination of care for all patients including multi-disciplinary coordination as a case manager.
    • Identify and recommend for referral to other services as indicated.
    • Assure patient safety.
    • Establish relationship of trust and confidentiality with patient/caregivers.
    • Practice Infection Control.
    • Meet obligation for reimbursement of the services.
    • Patient/Caregiver, Team & Community Education
    • Incorporate new information from ongoing dialogue with patient, family, team, physician and payer into treatment plan
    • Provide education/consultation to patient/caregiver/team/community.
    • Assess patient/caregiver learning needs, coordinate necessary teaching, and evaluate teaching outcomes.
    • Effectively identify and use appropriate educational tools and materials.
    • Plan educational programs to assist team/department with identified need.

    • Identify problems and/or actual or potential variances from goals and communicate with the health care team.
    • Initiate collaboration with the health care team to assure timely identification of the root cause(s) of problems/variances.
    • Participate in finding solutions for problems/variances and implement actions that move patients/caregivers toward desired goals.

    • Adapt communication to needs/level of patient/family/caregiver understanding or target audience.
    • Clearly and efficiently communicate significant information across the continuum of care and/or from one caregiver to another.
    • Independently collaborate with peers, members of HCT, and physician.
