Position Summary
The Home Health Intake Coordinator is responsible for managing the intake and referral process for home health services. This role serves as a key liaison between referral sources, patients, families, clinicians, and payers to ensure timely, accurate, and compliant admissions. The Intake Coordinator verifies eligibility, obtains authorizations, coordinates start-of-care, and ensures all documentation meets regulatory and payer requirements.
Key Responsibilities
- Receive, process, and manage incoming home health referrals from hospitals, physicians, facilities, and community partners
- Verify patient demographics, insurance eligibility, benefits, and authorization requirements
- Coordinate start-of-care services with clinical staff and scheduling teams
- Communicate effectively with patients, families, referral sources, and internal teams regarding admission status
- Ensure intake documentation is complete, accurate, and compliant with Medicare, Medicaid, and payer guidelines
- Enter and maintain referral and patient information in the EMR system
- Track referral outcomes and follow up on pending items (orders, authorizations, physician signatures)
- Identify and escalate issues related to coverage, eligibility, or incomplete documentation
- Maintain confidentiality and comply with HIPAA and agency policies
- Support quality improvement initiatives and intake workflow optimization
Qualifications
Required:
- High school diploma or equivalent
- Minimum 1–2 years of experience in home health, hospice, or healthcare intake/admissions
- Knowledge of Medicare, Medicaid, and managed care insurance verification processes
- Strong organizational and multitasking skills
- Excellent verbal and written communication skills
- Proficiency with EMR systems and Microsoft Office
Preferred:
- Associate’s degree or higher in healthcare administration or related field
- Experience with home health authorization and referral management
- Familiarity with CMS regulations and Conditions of Participation
