• Driver’s License
• Auto Insurance (if applicable)
• Health Certificate (physical exam and TB results within 6 months )
• Driver’s License

Training Certificates in:
• HHA/ CNA License
• BLS/CPR Card
• HHA/ CNA License
• BLS/CPR Card
• Alzheimer's Disease (2-hour course) Approved by the Department Of Elder Affairs.
• Assistance with Self-Administered Medications (2-hour course)
• Domestic Violence (2-hour case)
• HIPAA (2-hour course)
• HIV/AIDS (4-hour course)
• OSHA/Bloodborne Pathogens (2-hour course)
