Assisting Hands Home Care is seeking a compassionate and skilled Per Diem Registered Nurse (RN) / Nursing Supervisor to join our team. This role is responsible for overseeing client care, training and supervising caregivers, and ensuring high-quality service delivery.
Description:
- Conduct home visits to clients within designated geographic areas including but not limited to Bergen, Essex, Hudson, Hunterdon, Middlesex, Morris, Passaic, Union, & Warren counties.
- Follow individualized care plans tailored to each client’s needs
- Supervise and support CHHA (Certified Home Health Aide) and DSP (Direct Support Professional) staff
- Perform initial and ongoing client assessments and develop appropriate care plans
- Monitor client conditions and report any changes to the owner of the agency
- Provide case management and coordinate care across disciplines
- Accurately document all observations, interventions, and evaluations using a touch-screen tablet system
Requirements:
- Current RN license in good standing with the New Jersey Board of Nursing
- Proven experience in home care nursing and supervisory roles
- Exceptional interpersonal and communication skills
- Highly organized, detail-oriented, and capable of working independently
- Proficient in technology, including:
Microsoft Office Suite
Adobe
Scheduling software (preferably AxisCare)
- Comfortable working in a near-paperless environment
- Flexible and responsive to the needs of clients and the business
- Experience with accreditation survey preparation is a plus
