- Work one-on-one with an assigned patients (full-time 40 hour day or night shifts are available)
- Ensure quality and safe delivery of home health nursing services.
- Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs.
- Assist the RN in carrying out the plan of care.
- Graduate of accredited practical (vocational) nursing program.
- Licensed as an LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.
