- Complete initial and on-going interdisciplinary comprehensive assessments of the patient’s needs including OASIS assessments at appropriate time points.
- In partnership with the patient, representative (if any), and caregiver(s) identifies the physical, psychosocial, environmental, safety and developmental needs and initiates an individualized patient plan of care based on the needs identified.
- Revise the plan of care as necessary in partnership with the patient, representative (if any), and caregiver(s).
- Provide service and /or treatments requiring substantial and specialized nursing skills as ordered by the physician as indicated in the plan of care.
- Initiate appropriate preventative and rehabilitative nursing procedures.
Who you are:
- Graduate of an Accredited Nursing Program
- Licensed as an RN
- Licensed by the California Board of Registered Nurses
- Oasis charting experience preferred
