Licensed Vocational Nurse (LVN)
Join a team that brings care home.
We’re looking for a compassionate and reliable LVN to provide high-quality, patient-centered care in a home health setting. In this role, you’ll work closely with an RN and interdisciplinary team to support patients in the comfort of their own homes.
- Deliver skilled nursing care following individualized care plans
- Support the RN Case Manager with patient assessments and care coordination
- Monitor patient conditions and communicate updates to the care team
- Educate patients and families on self-care and treatment plans
- Perform clinical procedures within LVN scope of practice
- Maintain accurate and timely documentation
- Current California LVN license
- Minimum 1 year of recent nursing experience
- Current CPR certification
- Strong communication and organizational skills
- Ability to travel within the service area (valid driver’s license & insurance required)
- Physically able to assist patients with mobility and transfers
