We’re seeking a motivated and relationship-driven Sales Representative to connect patients, healthcare providers, and our agency’s Hospice services. The annual base salary range for this position at commencement of employment is expected to be between $80,000 - $95,000; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience. We also have uncapped commission and a $3,000 sign-on bonus!

What You’ll Do
- Build and maintain relationships with hospitals, physicians, case managers, and community partners
- Identify patient needs and coordinate appropriate home health, hospice, and caregiver services
- Act as a liaison between referral sources and the agency to support seamless patient transitions
- Educate healthcare professionals on agency services through meetings, presentations, and events
- Participate in discharge planning and care coordination discussions
- Execute territory plans, manage accounts, and consistently meet or exceed sales goals
- Collaborate with clinical and marketing teams to ensure effective service delivery and outreach
- Represent the agency at networking events and community activities
- High School Diploma required; Associate degree or higher preferred
- 2 years of outside sales or client service experience (healthcare experience a plus)
- Proven track record of meeting or exceeding sales goals
- Strong communication, relationship-building, and organizational skills
- Ability to work independently and manage a dynamic schedule
- Field-based role with local travel, including hospitals and community settings
- Requires standing, walking, and occasional lifting
- Must have valid driver’s license, auto insurance, and a safe driving record
