Medical Assistant - Chronic Care Management

Job OverviewWe are seeking a dedicated and experienced Medical Assistant with expertise in Chronic Care Management (CCM) to join our healthcare team in Dallas, TX. The ideal candidate will have hands-on experience supporting patients with chronic conditions, assisting providers in delivering comprehensive care, and ensuring effective management of chronic illnesses.
Key Responsibilities- Assist healthcare providers in managing patients with chronic conditions such as diabetes, hypertension, COPD, and others.
- Perform patient intake, including vital signs, medical history updates, and medication reconciliation.
- Coordinate and document Chronic Care Management services in compliance with CCM guidelines.
- Educate patients on disease management, medication adherence, and lifestyle modifications.
- Schedule follow-up appointments and facilitate communication between patients and providers.
- Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
- Support care coordination efforts to improve patient outcomes and reduce hospital readmissions.
- Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
- Proven experience in Chronic Care Management (CCM) within a clinical setting.
- Strong knowledge of chronic disease processes and management protocols.
- Excellent communication and patient education skills.
- Familiarity with electronic health records (EHR) and documentation standards.
- Ability to work collaboratively in a fast-paced healthcare environment.
- High level of professionalism and attention to detail.
