Responsibilities:
- Coordinate and manage the assessment and care planning process for residents, including the development and implementation of Individualized Care Plans
- Complete and submit accurate and timely MDS assessments, Care Plans, and other required documentation
- Collaborate with interdisciplinary team members to identify and meet the unique needs of each resident
- Provide education and support to residents, families, and staff on the MDS process and care planning
- Stay up-to-date with regulatory requirements and industry best practices to ensure compliance
Requirements:
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Missouri
- Strong understanding of long-term care services and regulations
- Excellent communication, organizational, and problem-solving skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
- Strong analytical and critical thinking skills
- Ability to maintain confidentiality and discretion when handling sensitive information
