About the Role:
As an Licensed Practical Nurse (LPN) at Aspire Senior Living Pleasant Hill, you will play a vital role in providing exceptional care to our residents. Your responsibilities will include:
Providing direct patient care, including administering medications, wound care, and vital sign monitoring
Developing and implementing individualized care plans to meet the unique needs of each resident
Collaborating with the interdisciplinary care team to ensure seamless transitions and comprehensive care
Maintaining accurate and up-to-date records of resident care and progress
Responsibilities:
- Assess and develop individualized care plans for residents
- Provide medication administration, wound care, and vital sign monitoring
- Collaborate with the interdisciplinary care team to ensure comprehensive care
- Maintain accurate and up-to-date records of resident care and progress
- Participate in care conferences and family meetings to ensure open communication
Requirements:
- Current Licensed Practical Nurse (LPN) license in the state of Missouri
- Associate's degree or higher in nursing preferred
- Experience working with geriatric population preferred
