About the Role:
As a Physical Therapist (PT), you will have the opportunity to make a meaningful difference in the lives of our residents. Your primary responsibilities will include:
Conducting initial and ongoing assessments of residents' physical abilities and developing individualized treatment plans
Providing physical therapy services to residents in our state-of-the-art therapy gym and in their residences
Collaborating with other healthcare professionals to ensure comprehensive care and achieve optimal outcomes
Educating and empowering residents and their families to take an active role in their care and recovery
Documenting patient progress and communicating with healthcare teams to coordinate care
