Responsibilities:
Provide direct patient care to our skilled nursing residents, including administering medications, wound care, and other treatments as prescribed by physicians
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Conduct routine vital sign checks, monitor for changes in condition, and report concerns to the Charge Nurse or Team Leader
Maintain accurate and detailed records of patient care and treatments
Participate in care conferences and strategic planning to ensure effective patient outcomes
Collaborate with other healthcare professionals to provide comprehensive care services
Requirements:
Current Licensed Practical Nurse (LPN) licensure in the state of Missouri
Ability to work a variety of shifts, including days, evenings, and weekends
Strong communication and interpersonal skills
Ability to work effectively as part of a team and prioritize tasks in a fast-paced environment
