Aspire Home Healthcare is seeking a licensed Physical Therapist (PT) to join our growing home health team serving
Typical Caseload
• Average 6 patient visits per day.
• Majority of the time the territory will be: Shasta county and Tehama county.
• Clinical support team assists with coordination and scheduling

Compensation
💰 $160,000 Annual Salary
💰 $15,000 Performance Bonus paid $5000.00 at 12,18 and 24 months.
💰 $5,000 Signing Bonus (paid after 90 days of employment)

Responsibilities
• Evaluate patients and develop individualized physical therapy treatment plans
• Provide therapy services according to physician orders and Plan of Care
• Monitor patient progress and update treatment plans as needed
• Educate patients and families on home exercise programs and safety
• Maintain accurate and timely clinical documentation
• Coordinate care with physicians and the interdisciplinary healthcare team
Qualifications
• Active California Physical Therapist (PT) License
• Home health experience preferred but not required
• Current CPR certification
• Valid driver’s license and reliable transportation
