We are currently hiring for a Social Worker position in the Salt Lake Valley Area.

JOB SUMMARY:

The Social Worker is responsible for the delivery of varied social work services to the Home Health and Hospice patient and family. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, bereavement services, and community education, outreach and referral. The Hospice social worker is an integral part of the Hospice IDT. The Social Worker also acts as a consultant to the agency staff to help them better understand the significant social and emotional factors concerning the patient.

QUALIFICATIONS AND EXPERIENCE:

1. Acceptable degree in Social Work froma CSWE accredited institution of Social Work. (SSW-LCSW)

2. Demonstrates ability in casework, counseling and group work, including one (1) year social work experience in a hospital, hospice agency, out-patient clinic, home health agency, medical rehabilitation, medical care or mental health program.

3. Valid license to provide services as SSW-LCSW in the State of Utah is required.

4. Valid Driver’s license, car insurance and car required.

5. Will be able to establish and maintain a good working relationship with the patient, patient’s family, and the personnel of the agency.

6. Has ready access to a telephone for receiving assignments or changes in assignments.

7. Must have grief or loss counseling education or experience.
