What are the responsibilities and job description for the Occupational Therapist position at Aspire Home Health Care Services?
We are a home health agency located in Cridersville, OH. We are looking for a, or multiple, Occupational Therapist(s) to help cover our ever growing patient base. We provide services to the following counties: Auglaize, Allen, Shelby, Logan, and Mercer. This is a a part-time or PRN position.
The OT will be responsible for completing admissions, 30-day evaluations and discharges. They will also be responsible for providing therapy services to our clients by following an individualized care plan approved by the patient's physician.
Qualifications:
-Valid OT license in the State of Ohio
-Valid driver's license
-Reliable transportation is a must
