Job Overview
Aspirational Health is seeking a compassionate and motivated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our preventive and lifestyle-focused primary care team. This position is well-suited for a starting nurse who is eager to learn, grow, and develop strong skills in outpatient medicine.
This is a highly patient-facing role requiring excellent communication, proactive outreach, and a caring, relationship-based approach. The nurse will support patient intake, counseling, chronic care management (CCM), remote patient monitoring (RPM), and preventive health workflows.
Key Responsibilities
Patient-Facing Care & Engagement
- Serve as one of the primary clinical contacts for patients.
- Provide warm, attentive patient intake including vital signs, medication review, health maintenance updates, and preventive screening assessment.
- Engage patients with supportive counseling, lifestyle guidance, and motivational coaching.
- Communicate clearly and calmly — even in challenging or emotional situations.
- Review discharge plans and ensure patients understand follow-up steps.
Proactive Outreach & Care Coordination
- Proactively call patients for follow-ups, care management check-ins, health coaching, and RPM monitoring.
- Maintain consistent engagement to support adherence, goal achievement, and symptom management.
- Help coordinate referrals, specialist visits, imaging, and follow-up appointments.
- Track open tasks and ensure follow-up is completed efficiently.
Chronic Care Management (CCM)
- Enroll eligible patients in CCM.
- Maintain and update individualized care plans.
- Perform monthly check-ins to review symptoms, goals, and barriers.
- Document progress and escalate concerns to the provider as needed.
Remote Patient Monitoring (RPM)
- Monitor daily and weekly readings, including:
- Blood pressure
- Blood glucose
- Weight
- Sleep metrics
- Identify abnormal values and alert the provider promptly.
- Educate patients on device use and lifestyle recommendations.
Clinical Procedures & Office-Based Care
- Start IVs (RN required; LPNs may monitor infusions depending on state scope).
- Administer injections and immunizations.
- Assist with office-based IV therapy and patient monitoring.
- Support exam room readiness and clinical flow.
Documentation & Workflow Management
- Document accurately in MEDENT EHR.
- Follow checklists, health maintenance trackers, and structured workflows with accuracy and attention to detail.
- Manage time efficiently to ensure smooth patient flow and timely completion of tasks.
- Maintain professionalism and HIPAA compliance at all times.
Required Skills & Qualifications
- Active RN or LPN license in Pennsylvania.
- Strong interpersonal and patient engagement skills.
- Willingness to proactively call and follow up with patients every day.
- Ability to communicate effectively in routine and difficult situations.
- Detail-oriented with strong checklist, data accuracy, and workflow management skills.
- Comfort with EHR systems (MEDENT experience preferred).
- For RNs: proficiency in IV insertion.
- For LPNs: willingness to learn IV monitoring and primary care workflows.
Preferred Qualifications
- Experience in primary care, outpatient medicine, or patient coaching.
- Familiarity with CCM and RPM programs.
- Interest in preventive health, lifestyle medicine, and patient motivation strategies.
