REPORTS TO: Director of Nursing
SUMMARY: Provides skilled professional nursing services to a caseload of patients in their place of residence in accordance with the patient’s plan of care.
DUTIES AND RESPONSIBILITIES:
- Assesses patients prior to provision of care.
- Provides ongoing assessment of the patient and the patient’s equipment needs.
- Initiates timely discharges.
- Educates and instructs the patient, patient’s family, or staff as required; counsels patient and family in meeting patient’s needs and goals.
- Prepares timely, legible, and complete documentation of all care, interventions, and care coordination as provided by law, regulation, and established policy; returns documentation to office/department according to established policy.
- Demonstrates an awareness of and sensitivity to patient/family rights.
- Applies safety principles as identified by established policy.
- Participates in the orientation and training of new staff.
- Participates in care coordination meetings.
- Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices.
- Ensures compliance with legal issues including but not limited to patient confidentiality and risk management; ensures compliance with JCAHO, federal, state, and local regulations.
- Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.
- Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.
- Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.
- Performs other related duties as assigned by management.
Requirements
QUALIFICATIONS:
- Must possess current state RN license and BLS certification.
- Minimum of one-year related experience or equivalent; infusion nursing experience is preferred.
- Must possess a valid driver’s license and auto insurance.
- Demonstrated competence in reacting to and handling emergencies.
- Strong charting/documentation skills.
- Ability to understand and adhere to established policies, procedures, and protocols.
