What This Role Is Really About
As a Hospice RN Case Manager, you’ll be the primary nurse guiding patients and families through their hospice journey. You’ll build trust, manage care with intention, and collaborate closely with an interdisciplinary team that truly supports one another.
What You’ll Do
- Manage a caseload of hospice patients with compassion and clinical excellence
- Perform routine nursing visits, assessments, and symptom management
- Educate and support patients and families through end-of-life care
- Collaborate with physicians, social workers, chaplains, CNAs, and leadership
- Participate in IDT meetings and care planning
- Ensure timely, accurate, and compliant documentation
- Advocate for patient comfort, dignity, and choice
What We’re Looking For
- Active Texas RN license
- Hospice or home health experience preferred (we will train the right nurse with the right heart)
- Strong assessment and critical-thinking skills
- Excellent communication and time-management abilities
- A compassionate presence and a heart for hospice
