Job Overview
Aspen Home Health and Hospice is seeking a compassionate, dependable, and skilled Registered Nurse (RN) to join our Home Health & Hospice team. This is a full-time opportunity for an RN who is passionate about providing exceptional, patient-centered care to individuals and families in the comfort of their own homes.
The Home Health & Hospice RN will provide skilled nursing services, assess patient needs, develop and implement individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure the highest quality of care.
Responsibilities
- Provide comprehensive nursing care to patients in their homes.
- Perform initial and ongoing patient assessments and document findings accurately.
- Develop, implement, and update individualized plans of care.
- Provide skilled nursing treatments and interventions as ordered.
- Monitor patient condition and communicate changes to physicians and the care team.
- Administer medications and treatments in accordance with physician orders and agency policies.
- Educate patients and caregivers regarding medications, disease processes, symptom management, and plan of care.
- Provide compassionate end-of-life and hospice care when appropriate.
- Support patients and families through the hospice process with dignity, respect, and empathy.
- Respond appropriately to changes in patient condition and urgent clinical needs.
Pay: $40.00 - $80.00 per hour
