Join our dedicated home health and hospice team as a Weekend On Call Registered Nurse (RN), where your expertise will provide vital patient care during evenings, nights, weekends, and holidays. As an on-call RN, you will respond promptly to urgent patient needs, coordinate care plans, and collaborate with interdisciplinary teams to uphold our commitment to exceptional patient-centered care. Responsibilities
- Provide immediate assessment and intervention for patients experiencing urgent or emergent health issues during on-call hours.
- Coordinate with physicians, family members, and caregivers to develop or adjust care plans based on patient needs and triage assessments.
- Administer medications, including IV infusions and injections, following aseptic technique protocols to ensure patient safety.
- Conduct comprehensive patient evaluations, including vital signs, physical examinations, and symptom assessments related to hospice or home health care.
- Document all patient interactions accurately within electronic health record (EHR) system, ensuring compliance with HIPAA regulations.
- Educate patients and families on medication administration, wound care, infection control practices, and disease management strategies.
- Collaborate with interdisciplinary teams—including social workers, therapists, and physicians—to ensure seamless continuity of care
Qualifications
- Current Idaho RN licensure in good standing.
- Experience in home health nursing and hospice care
- Strong clinical skills in IV infusion therapy, medication administration (including botulinum toxin treatment when applicable), suturing, and aseptic technique.
- Ability to perform comprehensive patient assessments, including vital signs, physiology knowledge, and physical examinations.
- Flexibility to work weekends, holidays, and overnight shifts as part of the on-call schedule; must possess basic life support certification.
Pay: $36.00 - $40.00 per hour
