Position Summary
Ask Health is seeking a Nurse Practitioner (NP) to join our physician-led neurology care team supporting facilities across the Northern New Jersey region. This role focuses primarily on in-person consultations and follow-up care the inpatient hospital setting, Skilled Nursing Facilities (SNFs) and other post-acute care settings.
The Nurse Practitioner will work closely with our neurologists and facility care teams to evaluate patients, manage neurological conditions, and provide ongoing consultative support for facilities requiring neurology expertise.
While prior neurology experience is preferred, it is not required. We welcome candidates with backgrounds in psychiatry, cardiology, stroke care, hospital medicine, internal medicine, or other acute care specialties. Key Responsibilities
Neurology Consults & Patient Care
• Conduct in-person neurological evaluations and consults for patients in hospital inpatient units and post-acute care facilities, including Skilled Nursing Facilities (SNFs), Long-Term Acute Care Hospitals (LTACHs), and rehabilitation centers.
• Perform initial consults and follow-up visits for patients with neurological conditions such as stroke, seizure disorders, cognitive decline, neuropathy, movement disorders, and other neurological complaints.
• Develop and implement clinical care plans in collaboration with supervising neurologists and facility care teams.
• Provide ongoing clinical monitoring, treatment adjustments, and follow-up care for patients requiring neurology management.
Facility Based Rounding
• Conduct scheduled in-person rounding at assigned facilities across the Northern New Jersey region.
• Evaluate new consult requests and follow-up patients requiring continued neurological oversight.
• Collaborate closely with facility medical directors, attending physicians, nursing teams, and care coordinators to support comprehensive patient management.
• Assist facilities in improving access to neurology expertise through timely consult availability and coordinated care.
• Build strong relationships with partner facilities and clinical teams to ensure high quality specialty care delivery.
Diagnostic & Treatment Management
• Order, interpret, and evaluate diagnostic studies including MRI, CT/CTA, EEG, and laboratory results
• Formulate differential diagnoses and initiate evidence-based treatment plans aligned with current neurology protocols
• Manage complex medication regimens including antiepileptics, anticoagulants, thrombolytics, stroke prevention therapies, and neuroprotective agents
• Monitor for therapeutic effectiveness and adverse effects, adjusting treatment plans accordingly
Collaboration & Care Coordination
• Collaborate closely with attending neurologists in diagnosis, treatment planning, and ongoing management of inpatient and outpatient cases
• Participate in interdisciplinary rounds with nursing, physical therapy, occupational therapy, speech therapy, and case management teams to ensure continuity of care
• Educate patients and families regarding disease processes, medication adherence, secondary stroke prevention, and lifestyle modification strategies
Required Qualifications
• Master’s or Doctorate degree in Nursing from an accredited Nurse Practitioner program
• Active and unrestricted New Jersey Advanced Practice Registered Nurse (APRN) license
• Current New Jersey Registered Nurse (RN) license
• National board certification as an Adult-Gerontology Acute Care NP (AGACNP), Family NP (FNP-BC), or Adult NP through ANCC or AANP
• Active DEA registration
• BLS and ACLS certification (AHA required)
• Proficiency in EMR documentation
Preferred Qualifications
• Minimum 1–2 years of neurology, stroke, or inpatient acute care experience preferred, but not required
• Experience with acute stroke protocols and hospital-based neurology consultation
• Familiarity with ENLS guidelines
• Skilled in managing complex neurological medication regimens
• Strong clinical judgment and ability to work independently within scope of practice
Work Environment
This position involves care delivery across inpatient clinics, telehealth platforms, inpatient hospital settings, and skilled nursing facilities. Schedule may include weekday coverage with occasional call responsibilities depending on service needs.
