Schedule and Compensation

Key Responsibilities
•Perform nutrition assessments for new admissions and quarterly reviews
•Develop and monitor individualized nutrition care plans
•Provide nutrition education and counseling as needed
•Collaborate with the interdisciplinary care team
•Ensure compliance with CMS, DOH, and applicable regulations
•Maintain accurate and timely documentation
•Monitor resident weights and nutrition risk indicators
•Conduct kitchen sanitation audits and review menus for USDA compliance
Qualifications
•Registered Dietitian Nutritionist (RDN) credential
•Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field
•Active state licensure (if required)
•Clinical experience preferred (SNF/LTC experience a plus)
•Strong communication and critical thinking skills
•Experience with electronic medical records
