About the Role
We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as a Clinical Evaluator – Program & Outcomes, supporting multiple programs including Unlimited Senior Solutions, Solara, and Mia Casa.
This role combines clinical evaluation, care coordination, and data tracking to support participant stability and demonstrate program effectiveness across community-based and housing programs.
Position Overview
The Clinical Evaluator is responsible for conducting wellness checks, evaluating prospective participants, coordinating healthcare services, and collecting data to measure program outcomes. This role works across multiple locations and plays a key part in ensuring participants are medically stable and appropriately connected to care.
Key Responsibilities
- Conduct wellness checks and ongoing health monitoring
- Evaluate prospective participants for eligibility and appropriate placement
- Identify health needs and connect participants to healthcare providers
- Support stabilization of medical and behavioral health conditions
- Coordinate with providers, case managers, and program staff
- Track and organize clinical and program data for reporting
- Assist in developing reports demonstrating program outcomes
- Maintain accurate, timely, and audit-ready documentation
Qualifications
- Active LPN or RN license in the State of Florida
- Strong clinical assessment and communication skills
- Ability to work independently across multiple sites
- Strong documentation and data tracking abilities
- Valid driver’s license and reliable transportation
Preferred Experience
- Community health, geriatrics, or case management
- Experience with senior populations
- Familiarity with Medicaid, Medicare, or waiver programs
- Experience in supportive housing or community-based programs
- Experience with reporting, outcomes tracking, or quality improvement
Technical Skills
- Microsoft Office (Word, Excel, Outlook)
- Comfortable with data tracking and reporting tools
- Ability to learn internal systems quickly
Pay: $21.00 - $24.00 per hour
