That’s why we also address food insecurity, youth leadership, and culturally competent care while working towards supporting safer streets and vibrant communities.Location: Rolland & Kathryn Lowe Medical Center (RKLMC)Job Summary: Asian Health Services (AHS) is seeking a Geriatrician to join our team at a Federally Qualified Health Center (FQHC) as part of our Senior Access and Geriatric Expansion (SAGE) clinic. The ideal candidate will be passionate about providing high-quality care to elderly populations, particularly those facing complex health challenges. You will work within a multidisciplinary team to ensure that our senior patients receive comprehensive, culturally competent care aimed at improving their quality of life.Essential Job FunctionsProvide both primary and consultative direct medical care to geriatric patients, including assessment, diagnosis, treatment, and management of both acute and chronic conditionsLead the SAGE interdisciplinary team, which also includes a geriatric nurse and community health worker, to provide support for patients and families on addressing unique complex medical and social issues Conduct routine screenings, preventive care, and health assessments to identify issues such as frailty, cognitive decline, polypharmacy, functional limitations, and goals of care.
