That’s why we also address food insecurity, youth leadership, and culturally competent care while working towards supporting safer streets and vibrant communities.Location: Rolland & Kathryn Lowe Medical Center (RKLMC)Job Summary: Asian Health Services (AHS) is seeking a Geriatrician to join our team at a Federally Qualified Health Center (FQHC) as part of our Senior Access and Geriatric Expansion (SAGE) clinic. The ideal candidate will be passionate about providing high-quality care to elderly populations, particularly those facing complex health challenges. You will work within a multidisciplinary team to ensure that our senior patients receive comprehensive, culturally competent care aimed at improving their quality of life.Essential Job FunctionsProvide both primary and consultative direct medical care to geriatric patients, including assessment, diagnosis, treatment, and management of both acute and chronic conditionsLead the SAGE interdisciplinary team, which also includes a geriatric nurse and community health worker, to provide support for patients and families on addressing unique complex medical and social issues Conduct routine screenings, preventive care, and health assessments to identify issues such as frailty, cognitive decline, polypharmacy, functional limitations, and goals of care. Work collaboratively with outside specialists and other interdisciplinary team members across the organization (nurses, social workers, care coordinators, mental health providers, etc) to deliver person-centered careAdvocate for seniors’ health and wellness needs within the community and assist in navigating social services and community resourcesMaintain accurate and timely documentation of patient care using electronic health record (EHR) systemsAdhere to all FQHC regulations, standards, and guidelines to ensure compliance and optimal care deliveryLeadershipProvide mentorship and support to other clinical staff and trainees, fostering an environment of learning and professional growth around geriatric issuesParticipate in quality improvement activities and collaborate with the leadership team to enhance patient outcomesParticipate in the development and implementation of new geriatric care protocols and best practices in primary care Serve as a geriatric liaison to the AHS Program for All-Inclusive Care for the Elderly (PACE) for transitioning care of patients from FQHC to PACE Provide as needed back up coverage and support for future PACE program Minimum QualificationsMD or DO with board certification/eligibility in internal medicine or family medicine with at least 2 years of experience in caring for geriatric adults Valid state medical license and DEA registration in California Basic Life Savings (BLS) certification Strong communication and interpersonal skills, with the ability to establish rapport and trust with older adults and their families.Commitment to providing high-quality, compassionate care to underserved populations.Preferred QualificationsFellowship training in Geriatrics Bilingual in Chinese (Mandarin or Cantonese), Vietnamese, and/or any Asian languages Proficiency with EPIC (EHR).Experience in managing an interdisciplinary teamExperience in a community health or federally qualified health center setting Familiarity with PACE or other types of value-based models of care for seniors Benefits: Medical, Dental & Vision premiums 100% paid by AHSAcupuncture & Chiropractic coverage12 Vacation Days12 Sick Days12 Holidays 3 Floating Holidays3% 403(b) Employer Contribution up to 2% Employee MatchCommuter BenefitsFlexible Spending Account (FSA) & Dependent Care AssistanceLong-Term Disability InsuranceWe may use artificial intelligence (AI) tools to support parts of the hiring process, such as reviewing applications, analyzing resumes, or assessing responses.
