- Pay range is between $29 to $32 hourly. Can increase based on experience with possible increases after 6 months from date of hire.

- Conducts basic assessment of social and health needs for walk-ins and new referrals. Creates person-centered service plan identifying key needs, barriers, and potential service referrals. Delivers services based on plan and coordinates care by facilitating connections to services, addressing access barriers, and providing social support. Provides client advocacy in attaining services and assistance in areas such as housing, medical resources, benefits enrollment, financial assistance, legal advocacy, personal care service, transportation, translation/interpretation. Conducts follow-up to ensure successful linkages to needed services.
- Conducts community outreach and education to raise awareness about available resources and ways to access services.
- Prepares concise, complete, and accurate program documentation according to agency policy and system standards and in a timely manner. Maintains complete and accurate case files and records. Meets program documentation and client confidentiality per agency policy.
- Participates in internal team meetings and external partnership meetings in coordination with supervisor. Coordinates and cooperates with other agencies and organizations to ensure quality care to clients.
- Attends all agency, program and team meetings, in-service trainings, and supervision.
- Other job-related duties as assigned by management.
- Bachelor's degree in social work or social/behavioral science; OR
HS diploma or equivalent, plus five (5) years of social services experience.
- Ability to provide effective, respectful, culturally responsive care and services. Experience working with marginalized communities. Prefer fluency in Asian language. Lived or community experience is valued.
- Experience in navigation of health and social services delivery system.
- Strong communication skills and ability to work effectively as a team member with professional and support staff; apply case management best practices and principles to individual situations; and exercise judgment and organize multiple tasks.
- Must have reliable transportation. If using a personal or an agency vehicle to fulfill job duties, must have a valid policy. If using a personal vehicle, must have current auto insurance including coverage for work-related business.
- Effective ability to navigate a personal computer. Competency in use of the Microsoft Office and other relevant software for word processing, calendaring, e-mail, and data entry. Efficiently research and perform various tasks via the internet.
