Social Care Coordinator (Billigual Spanish)

Position Overview:

Asian Community Care Organization (ACCO), a nonprofit Community-Based Organization (CBO), is seeking a Social Care Coordinator to connect Medicaid populations in Manhattan, Queens and Brooklyn to essential social supports. The role involves screening and assessing health-related social needs under the New York State 1115 Waiver Social Care Network (SCN) program, and facilitating access to food, housing, and transportation resources using an online referral platform.

Key Responsibilities:

· Conduct outreach and screen Medicaid populations for social needs.

· Refer clients to appropriate community-based services and follow up to ensure needs are met.

· Support clients with Medicaid applications, including occasional in-person visits to Medicaid offices across NYC to assist with submission.

· Document activities and manage workflows using SCN’s IT platform.

· Collaborate with internal teams, and SCN Lead Entities to ensure seamless coordination of services.

· Assist in community outreach and education efforts to expand the reach of SCN services and improve program participation.

· Assist with training and reporting using SCN tools and platform.

· Other duties as assigned by the supervisor that include logistical and programmatic support.

Qualifications:

· 1-2 years’ experience in care coordination or social services.

· Strong organizational, problem-solving, and communication skills.

· Proficiency in using technology platforms and engaging with diverse populations.

· English fluency and a second language (e.g., Chinese, Korean, Spanish, Russian, etc.) required.

· Bachelor’s degree in public health or social work preferred.
