Job Overview
Ashram Adult Day Health Care is seeking a compassionate, organized, and dependable Master of Social Work (MSW) to join our Community-Based Adult Services (CBAS) team.
The Medical Social Worker will provide psychosocial assessments, person-centered care planning, case management, supportive counseling, community-resource coordination, and family support for older adults and adults with disabilities. The MSW will work closely with participants, caregivers, nursing staff, therapists, activity staff, dietitians, physicians, health plans, and community agencies.
This position requires accurate and timely documentation using CorreLink and active participation in the center’s multidisciplinary team.
Primary Responsibilities
- Interview and screen prospective participants to determine their general appropriateness for CBAS services.
- Complete initial psychosocial assessments, reassessments, quarterly progress notes, and other required social-service documentation.
- Participate in multidisciplinary team meetings and contribute to each participant’s individualized, person-centered plan of care.
- Develop measurable social-service goals and interventions based on participants’ assessed needs, strengths, preferences, and support systems.
- Provide case management, supportive counseling, crisis support, advocacy, and problem-solving assistance within the MSW’s scope of practice.
- Connect participants and caregivers with community resources, including housing, transportation, food assistance, Medi-Cal, Medicare, IHSS, behavioral-health services, caregiver support, and other programs.
- Serve as a liaison between participants, families, caregivers, physicians, health plans, community providers, and the CBAS multidisciplinary team.
- Monitor participants for changes in physical, cognitive, emotional, behavioral, or social functioning and communicate concerns to the licensed nurse and other appropriate team members.
- Provide referrals for mental-health evaluations or other specialized services when indicated.
- Facilitate supportive or problem-centered discussion groups that promote social interaction, coping skills, and emotional well-being.
- Complete and document required follow-up on participant and family needs.
- Coordinate and document discharge planning, referrals, and transition services.
- Recognize and report suspected abuse, neglect, exploitation, or unsafe living conditions in accordance with mandated-reporting laws.
- Protect participant rights, privacy, dignity, choice, and confidentiality.
- Participate in staff meetings, training, quality-assurance activities, chart reviews, and regulatory surveys.
CorreLink and Documentation Responsibilities
- Maintain accurate, complete, signed, dated, and timely participant records in CorreLink.
- Document assessments, reassessments, quarterly progress reports, care-plan goals, interventions, case-management services, referrals, follow-up contacts, family communications, significant changes, and discharge planning.
- Complete documentation according to CBAS, Medi-Cal, managed-care-plan, HIPAA, and center requirements.
- Review participant records regularly and correct incomplete or inaccurate documentation promptly.
- Assist with preparing social-service records for audits, surveys, and managed-care-plan reviews.
Required Qualifications
- Master’s degree in Social Work from an accredited institution.
- Strong assessment, case-management, communication, and documentation skills.
- Ability to work effectively with older adults, adults with disabilities, families, caregivers, and a multidisciplinary healthcare team.
- Ability to maintain confidentiality and comply with HIPAA requirements.
- Basic computer proficiency and ability to learn and use CorreLink.
- Ability to complete all required background clearances, health screenings, and TB testing.
Preferred Qualifications
- Punjabi language proficiency.
- At least one year of experience in CBAS/ADHC, aging services, healthcare, case management, behavioral health, or long-term care.
- Experience working with older adults, individuals with disabilities, dementia, cognitive impairment, or chronic medical conditions.
- Familiarity with CBAS assessments, Individual Plans of Care, quarterly progress reports, and discharge planning.
- Experience using CorreLink or a similar electronic health-record system.
- Knowledge of Medi-Cal, Medicare, IHSS, managed-care plans, and community resources.
Licensure
An LCSW license is not required for this position unless the employee is assigned services that legally require clinical licensure. Unlicensed social workers must practice within their education, training, competence, and permitted scope of practice.
