What are the responsibilities and job description for the Clinical Pharmacist - Acute Care position at Ashmore Connection?
Ashmore Connection is currently seeking a Clinical Pharmacist to support an acute care healthcare setting. The Clinical Pharmacist will provide comprehensive medication management, collaborate with physicians and healthcare teams, and help ensure safe, effective, evidence-based medication use.
Responsibilities:
- Review and optimize medication therapy for acute care patients
- Collaborate with physicians, nurses, and other healthcare professionals
- Monitor patient medication regimens, drug interactions, dosing, and therapeutic outcomes
- Provide clinical recommendations and medication-related education
- Support medication safety and quality improvement initiatives
- Maintain accurate documentation in accordance with facility and regulatory requirements
Qualifications:
- BS in Pharmacy or Doctor of Pharmacy (PharmD) or equivalent pharmacy degree
- Active Florida Pharmacist License or ability to obtain
- 1 Year of Acute care or hospital pharmacy experience preferred
- PGY1 Residency Preferred
- Strong communication skills
