Hospice Social Worker – Make a Lasting Difference
Pay up to $32 per hour (DOE) mileage reimbursement

As a Hospice Social Worker, you’ll be the emotional anchor for patients and families—helping them navigate the practical, emotional, and spiritual challenges that come with life-limiting illness.
You’ll provide counseling, connect families to vital resources, advocate for their needs, and collaborate closely with our nurses, physicians, and spiritual care team. Every day, your work will make a direct impact on someone’s comfort, peace, and well-being.
What You’ll Do
- Provide psychosocial assessments and bereavement support for patients and families.
- Offer short-term crisis intervention and ongoing emotional support.
- Assist families in accessing resources like Medicaid, insurance benefits, and community programs.
- Advocate for patients’ needs while collaborating with the full care team.
- Maintain timely, accurate documentation of visits and referrals.
- Participate in bereavement programs, volunteer training, and community outreach.
Qualifications
- Required: Bachelor’s degree in Social Work (CSWE-accredited) and active state Social Work license.
- Preferred: Master’s degree in Social Work (MSW) and hospice or medical social work experience.
- Minimum 1 year in hospital, hospice, home health, mental health, or related setting.
- Strong communication skills—both written and verbal.
- Valid driver’s license, reliable transportation, and current auto insurance.

Work Environment
This is a field-based and office-based role with local travel to patient homes and facilities. Some evenings and weekends may be needed to meet urgent patient and family needs.
