Join our compassionate team as a full-time RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. At Auburn Crest Hospice our mission is to create a culture of excellence that honors life by empowering employees, patients and communities to live every moment!
What You'll Do:
- Manage 10-14 patients, ensuring they receive appropriate care.
- Complete assessments, create care plans, and document care.
- Collaborate with doctors, social workers, and other team members.
- Provide support and education to families.
- Enjoy flexible scheduling and work-life balance (avoid burnout!).
We're Looking For:
- Idaho RN License.
- 2 years of experience (pref. hospice).
- Strong communication and teamwork skills.
- Valid driver's license & reliable transportation.

Certifications, Licenses and Registrations
- Registered Nurse licensed in the state of employment.
- Minimum of one (1) year nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years is preferred.
- Certification in Hospice and Palliative Nursing a plus but not required.
- Current CPR certification.
- Must have a valid driver’s license and auto liability insurance.
