Children's Long-Term Support (CLTS) & Children's Community Options Program (CCOP) Case Manager
The Ashland County Health & Human Services Department is seeking a qualified professional to provide case management and service coordination for children with physical, developmental, or emotional disabilities through Children's Long-Term Support (CLTS) and Children's Community Options Program (CCOP). This position works collaboratively with children, families, providers, schools, medical professionals, and community partners to identify needs, develop service plans, coordinate supports, and promote positive outcomes that help children remain safely in their homes and communities.
Responsibilities include:
- Develop and monitor individualized service plans in partnership with families and interdisciplinary team members.
- Coordinating and authorize services in compliance with CLTS and CCOP guidelines
- Assist families in accessing community resources, benefits, and supports
- Maintaining accurate documentation and collaborating with partner agencies
Minimum Qualifications:
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Special Education, Nursing, or a closely related field.
- Valid Wisconsin driver's license, insurance, and reliable transportation
- Strong organizational, communication, and documentation skills.
- Ability to work independently while collaborating effectively with families and community partners.
Salary and Benefits:
- $23.18 - $25.17 per hour, based on education and experience
- Excellent benefits package, including a 4-day flexible work week (35 hours), 2 weeks of paid vacation after 90 days, health/life/dental insurance, and participation in the Wisconsin State Retirement System
