Ashford Hall, an established and rapidly growing skilled nursing facility, is seeking a qualified, experienced MDS Coordinator (LVN orRN) for full-time employment. The primary purpose of your position is to conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment. The successful candidate will have excellent communication skills and be able to work collaboratively with other healthcare professionals.

Reports to the Administrator
Responsibilities:
- Assess resident's condition as it relates to documented notes in the electronic medical record
- Plan and complete resident assessments and MDS' timely and completely
- Coordinate with IDT to ensure each section of the MDS is completed timely and accurately
- Communicate with appropriate team members and provide education and support
- Collaborate with other healthcare professionals to coordinate care and ensure continuity of treatment
- Stay up-to-date with current MDS rules and regulations and best practices in nursing
- Participate in professional development opportunities to maintain and improve skills
- Other duties as assigned
- Bachelor's degree in Nursing preferred, but not necessary
- Current Texas state license as a Registered Nurse or Licensed Vocational Nurse
- Minimum of 3 years of experience as an MDS Coordinator in a Medicaid/Medicare Skilled Nursing Facility - Long Term Care Facility
- Excellent communication and interpersonal skills
- Ability to work collaboratively with other healthcare professionals
