Position Title: Intake Coordinator—SSVF Department/Program: Veterans Services of the Carolinas (VSC) Reports To: Assistant Director VSC Operations

Location: 1845 Brevard Road Arden, North Carolina 28704

FLSA Status: Non Exempt

Work Schedule: Full-time position requiring flexibility, including occasional overtime.

Compensation: Hourly rate commensurate with education and experience.

POSITION SUMMARY

The Intake Coordinator – SSVF serves as the initial point of contact for veterans and veteran families seeking assistance through the Supportive Services for Veteran Families (SSVF) program. This position coordinates participant intake, eligibility screening, referral management, and administrative support while ensuring timely access to services and accurate documentation in compliance with program requirements.

ESSENTIAL DUTIES & RESPONSIBILITIES

Intake & Eligibility Coordination

 Serve as the initial point of contact for veterans and veteran families seeking SSVF services.

 Conduct intake interviews, eligibility screenings, and preliminary assessments in accordance with program requirements.

 Collect, review, and verify documentation related to veteran status, income, housing status, and program eligibility.

 Explain available services, referral processes, and program expectations to participants.

Referral Coordination & Systems Navigation

 Coordinate referrals with case managers, healthcare providers, behavioral health agencies, VA partners, housing organizations, and community resources.

 Assist participants in navigating housing resources, healthcare systems, supportive services, and community programs.

 Maintain current knowledge of community resources, referral systems, and veteran-serving organizations.

 Support participant follow-up and continuity of care.

Documentation & Administrative Support

 Maintain timely and accurate participant records, intake documentation, eligibility determinations, referrals, and program files.

 Utilize HMIS, Unite Us, and other required databases and reporting systems.

 Ensure compliance with grant requirements, confidentiality standards, and organizational policies.

 Assist with reporting, audits, file reviews, and quality assurance activities.

Team Collaboration & Ministry Support

 Collaborate with case managers, peer support specialists, healthcare providers, churches, volunteers, VA personnel, and community organizations.

 Participate in outreach activities, staff meetings, training, and ministry initiatives.

 Promote ABCCM's mission and values in all participant and community interactions.

 Perform other duties as assigned.

QUALIFICATIONS

 High school diploma or GED required; additional education in Human Services, Social Work, Public Administration, Behavioral Health, or a related field preferred.

 Experience in intake coordination, case management support, veteran services, and homelessness services, healthcare, or related human services preferred.

 Experience using HMIS, Unite Us, or similar databases preferred.

 Strong computer, documentation, and reporting skills required.

 Experience working with vulnerable populations preferred.

 Valid North Carolina driver's license preferred.

PHYSICAL REQUIREMENTS / WORKING CONDITIONS

Work is performed primarily in office, community, healthcare, and outreach environments throughout Western North Carolina. Responsibilities require regular computer use, telephone communication, prolonged sitting, standing, walking, operation of standard office equipment, and occasional travel. The position regularly interacts with veterans and families experiencing homelessness, trauma, behavioral health conditions, financial hardship, and other complex service needs.

MENTAL DEMANDS

Must be able to manage multiple participant inquiries, intake activities, eligibility determinations, referral coordination, documentation requirements, and changing priorities simultaneously. Requires strong organizational, communication, problem-solving, emotional resilience, and sound judgment while serving vulnerable populations.
