Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by taking vital signs such as blood pressure, pulse, and temperature; report any concerns promptly
- Support medication administration following care plans and HIPAA privacy regulations to ensure confidentiality and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Conduct patient observation to identify changes in condition or behavior, especially for those with dementia or Alzheimer's disease
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and laundry
- Use specialized equipment like Hoyer lifts or resident’s
to safely transfer individuals with mobility challenges
Requirements
- Prior experience in home health, assisted living, nursing home, or group home settings is highly preferred
- Knowledge of patient care techniques including assisting with ADLs, medication administration, and patient monitoring
- Certifications such as CPR, First Aid, and HIPAA compliance are required; additional training in dementia or Alzheimer's care is a plus
- Ability to work with individuals with developmental disabilities, autism spectrum disorder, or other special needs
- Strong communication skills to effectively collaborate with healthcare teams and family members
- Physical stamina for heavy lifting and assisting residents with mobility needs using equipment like Hoyer lifts
- A caring attitude combined with patience, empathy, and a commitment to providing exceptional patient-centered care
