Responsibilities
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems
- Support medication administration following proper protocols and respecting patient confidentiality in compliance with HIPAA regulations
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care and behavior management strategies
- Assist with medical procedures such as catheter care, wound dressing, and hospice/end-of-life support while ensuring patient comfort and safety
- Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and meal preparation in accordance with home & community care standards
- Document patient progress meticulously and communicate effectively with nursing staff regarding any concerns or changes in condition
Skills
- Proven experience working with geriatric patients or individuals with developmental disabilities in settings such as nursing homes or assisted living facilities
- Strong knowledge of medical terminology, medication administration, and basic life support (CPR/First Aid) techniques
- Familiarity with EMR/EHR systems for accurate medical recordkeeping and documentation
- Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or wheelchairs
- Excellent patient observation skills to detect changes in condition or behavior promptly
- Compassionate caregiving approach tailored to dementia care, hospice needs, or end-of-life support
- Knowledge of home healthcare regulations and best practices for delivering high-quality home & community care
