Role Description
We are seeking a full-time on-site Manager of Case Management to join our team in Evansville, IN. In this role, you will oversee and coordinate case management functions, ensuring patients receive high-quality, appropriate, and cost-effective care. Key responsibilities include managing discharge planning, facilitating effective utilization of healthcare resources, mentoring case management teams, and collaborating with interdisciplinary teams to optimize care plans. You will also handle the development and implementation of managed care strategies to ensure compliance with regulatory requirements and best practices.
Qualifications
- Expertise in Discharge Planning and Case Management
- Possession of Certified Case Manager credential is preferred
- Strong leadership, communication, and problem-solving skills
- Experience in healthcare regulatory compliance and patient advocacy
- Bachelor’s degree in Nursing, Social Work, or a related field required; Master’s degree preferred
- Active and valid professional licensure in Indiana or eligibility to obtain licensure
