At Ascension Rehab Care, we deliver 1:1, evidence-based physical therapy in the places our patients live and thrive—private residences, senior living communities, and residential care facilities across South Carolina.
We’re looking for a licensed Doctor of Physical Therapy (DPT) who values patient-centered outcomes, clinical autonomy, and real-world functional rehabilitation.
What You’ll Do
- Perform comprehensive PT evaluations and reassessments
- Design and update individualized, outcome-driven plans of care
- Deliver skilled interventions including:
- Manual therapy, trigger point release, and myofascial techniques
- Balance, gait, and functional mobility training
- Therapeutic exercise and neuromuscular re-education
- Post-operative orthopedic rehabilitation protocols
- Pain science and progressive mobility programs
- Provide caregiver and patient education for long-term independence
- Maintain timely, compliant EMR documentation
- Coordinate care with physicians and interdisciplinary clinical partners
- Support safe, structured discharge planning
- Clinically supervise PTAs per state and payer guidelines

Requirements
- Active, unrestricted Physical Therapist license in South Carolina
- Accredited Doctor of Physical Therapy (DPT) or equivalent degree
- Valid NPI (Type 1)
- Current CPR/BLS certification
- Strong clinical judgment and ability to work independently in a mobile model
- Excellent communication and caregiver engagement skills
- Commitment to compliance and high clinical quality standards
