Clinical Quality Improvement Coordinator
Hours: 8:30AM - 5:00 PM
Location: NJ
The Clinical Quality Improvement Coordinator will be responsible for a variety of auditing, process improvement, and consultation duties in support of clinical operations across Ascend Hospice. This role develops, plans, implements, and coordinates clinical data collection and analysis. The Coordinator collaborates with agency management, clinicians, and corporate clinical leadership to improve clinical outcomes through the QAPI process. This position also recognizes and performs essential duties even when not directly assigned, in accordance with regulatory and organizational standards.

Key Responsibilities Include:
- Develops, plans, implements, and coordinates the agency’s clinical data collection plan and analysis
- Performs agency audits within required timeframes and compiles data from the auditing process and publicly reported sources
- Collaborates with leadership to implement and support the Agency QAPI plan
- Develops and evaluates clinical education based on audit findings, surveys, and agency needs
- Reports audit results through the QAPI process up to the Clinical Center of Excellence Team and Governing Body
- Monitors performance improvement plans and ensures identified issues are addressed
- Responds promptly to concerns, develops corrective actions, and reports findings
- Supports the education and orientation of new hospice clinicians and provides one on one training when needed
- Assists with external audits and identifies areas for targeted performance improvement
- Maintains and documents competencies and licensure
- Participates in continuing education, shares expertise with co workers, and supports a collaborative work environment
- Encourages diversity among staff and actively participates in identifying and solving problems

Education Requirements:
- Associate’s Degree in Nursing required
- Bachelor’s Degree preferred
- Licensed Registered Nurse in the state of practice
- Certified Hospice and Palliative Care Nurse CHPN is a plus
- Minimum of two years of hospice clinical care experience
- Knowledge of hospice regulations, eligibility, and clinical operations
- Strong verbal and written communication skills
- Proficiency with Microsoft Windows and Office applications
- Ability to motivate and educate teams toward continuous process and quality improvement
